Financial Policy
Thank you for choosing Snap as your health care provider. Our goal is to provide you with quality medical care. To make our services available to as many patients as possible on an affordable basis, we have adopted the collection policy outlined below. We ask that you read the policy carefully and contact us with any questions.
Snap Diagnostics may accept any assignable insurance with applicable coverage.
- Payment is typically charged to your credit card on file after testing has been completed and processed by your insurance. If necessary, you can also review your bill and manage payment online or by mail.
- If your insurance is not accepted, or you plan to self-pay for the test, full payment is due once we send your test results to your medical provider.
- Payment is accepted by cash, check, credit or debit card, Health Savings Account (HSA), or flexible spending account (FSA) card.
- Extended payment plans are available with prior approval.
Regarding Insurance
We may accept assignment of insurance benefits at our discretion if acceptable insurance identification is provided. Acceptable insurance identification is defined as a valid insurance card, policy/plan with applicable coverage, or telephone verification. As a courtesy to our patients, verifiable and assignable insurance will be billed by Snap Diagnostics. However, you will be personally responsible for your account balance regardless of whether or not your insurance will pay for the total balance of your claims. Your insurance policy is a contract between you and your insurance provider. Snap Diagnostics is not a party to that contract.
If you choose or are required to bill your own insurance, Snap Diagnostics will provide an itemized statement and a HCFA-1500 Form but will treat the account as a self-pay.
